Compliance With Texas House Bill 4224

Effective September 1st, 2025. In alignment with Health and Safety Code Section 181.105 and Texas House Bill 4224 (89th Regular Session), we are required to post details regarding how to:

  1. a) contact the state's behavioral health executive council (BHEC),
  2. b) filing a consumer complaint, and
  3. c) requesting records from your mental health provider.

While Canopy Counseling & Well-Being includes all of this information, and more, in client paperwork, posting this detailed information on our website complies with the new requirements.

Business Violations: Consumer Complaint

If you would like to file a consumer complaint against a business in the state of Texas please contact the Texas Attorney General’s Office to do so.

Requesting Mental Health Records

You have the right to request a copy of your mental health records. Records will be provided in accordance with Texas law and applicable privacy regulations. Records can be requested via email or in writing. A release of information (ROI) must also be completed.  Furthermore, the client is charged a $2.00 per-page copying fee, and there may be an additional fee for accessing records.

 Please make your request well in advance and allow 2 weeks to receive the copies. 

 Please note that, under certain limited circumstances, as listed in 45 CFR 164.524(a)(2) – (a)(3), we may deny access to all or a portion of the PHI requested.
